The files posted so far have been created from the starting point of knowing what the pocketed flat area size is and working outwards from there. Sometimes, though, you know what you want for the outside of the cut (i.e. surface level).
I've taken dealguy's file (his solution is on the right) and added toolpaths and vectors (on the left) that starts outside and works in.
Note that when created with a closed vector the moulding toolpath wants to cut on the outside of that vector. To get around that I've split the closed vector into two open vectors. Then I created a moulding toolpath for each of the closed vectors. I controlled the side the toolpath cuts on by reversing the rail or profile's direction as necessary.

PS. If you are not ready to upgrade to v11.5 yet you could download a trial version of either VCarve or Aspire 11.5. That way you could be able view files posted in versions later than v9.519.
