You are right. Without the two Tool Option, it works fine. Is it a bug? You should not be able to rotate the nut at all.

You have definite machine problems, not software troubles.
It appears the machine has backlash and/or some flexing issues.
Also, it appears that the angle of the v bit may not be the same as specified in the tool database.
