It's a contour shuttle pro, they use them to edit movies mostly but mach3 was kind enough to support the plugin to make it programmable, you can set any function/macro/script to any key on it. Copy G54 from G59.253 on startup

Also you have persistent DROs unchecked. Maybe I best post this on the mach3 forum, some of those CV settings I haven't a clue what they mean/do.
