Using router bits wuth a pilot bearing?
Can you create tools in the database that account for an edge following pilot bearing? I understand that I'd have to machine relief for the bearing.

Why would you want to use a guide bearing on a CNC ? It will either do no good at all (because the bearing isn't touching the guide surface) or it will be trying to bend/break your bit because it is in contact with the guide surface and/or keep your tool from going to depth you want.
You should remove the bearing ASAP.

Only because such bits are readily available for things like a 3/8" radius round-over, for example.
The bearing can be removed, of course that still leaves the boss that the bearing was mounted to. Even that can be ground away leaving an area on the bottom face of the bit that is not a cutting surface.
I saw this video where it's being done (they are using Aspire) but he doesn't go into creating the tool in the database.

As has been pointed out it's much better to use the tools designed for the purpose where possible though.

In most cases, bits with installed bearings are designed for side cutting only. bearing on or off, they will not plunge into material like the ones shown in links above. There are dozens if not hundreds of proper bits to do the job you require. Get the right bit.
