I've got a form tool setup in the database which has a profile as below:
However when I run the 3D preview I just get the following result:
The depth of the cut in relation to the tool are correct and it's only doing a single pass. I get the same problem with all Form tools regardless of their shape however a vbit renders as expected. What am I doing wrong?

It would be a horrendous task to write the software to deal with "undercuts" when simulating a cut. A dovetail bit is a good example of this and the waist area of your form is another.

Can I create a tool with an undercut?
No, the Form Tool will not allow a bit with an undercut to be modeled since toolpaths using bits with undercuts cannot be previewed.
If you need to use a bit with undercut in a project, you can create a toolpath using a standard bit but run that at your machine with the undercut bit installed.
For example, you want to create a T-slot using a T-slot bit. You can create a toolpath using an end mill and preview that, realizing at your machine there will be a horizontal slot cut at the bottom of the toolpath. To do this safely and successfully, you need to have a good understanding of toolpaths, how to set up a single pass toolpath and possibly the use of Lead-ins and Lead-outs.

Vectric products model the material and cuts as surfaces. Since only the surface (top) is represented, there is no way an undercut can be handled as such by the program.

I'm surprised you were able to make a form tool in that shape to start with.
